Work Environment

This position is based in Savannah, Georgia, a vibrant city known for its historic charm and picturesque landscapes. You will work in an onsite facility that fosters a collaborative healthcare environment, free from the usual constraints of high patient turnover and billing pressures found in traditional practices. The schedule is structured for optimal work-life balance, with no weekends or on-call duties, and you will typically work Monday, Wednesday, and Friday from 8:00 AM to 5:00 PM.

Key Responsibilities

• Conduct thorough health assessments and evaluations for patients, ensuring personalized care tailored to individual health needs.
• Perform onsite health risk assessments to proactively identify potential health issues and create effective intervention plans.
• Educate patients on wellness, preventive measures, and healthy lifestyle choices, fostering long-term health improvement.
• Support patients in managing chronic conditions, developing treatment plans, and providing continuous monitoring to optimize disease management.
• Oversee care coordinated by the medical team, ensuring seamless communication and collaboration among staff members.

Qualifications

Compensation and Benefits

The salary for this full-time Family Nurse Practitioner position ranges from USD 125,000 to 140,000 annually, complemented by a robust benefits package.
