Key Responsibilities\n

As a Clinical Licensed Professional Counselor, you will take on a variety of responsibilities, including:

• Conducting thorough patient intakes, which focus on diagnostic evaluations and functional assessments, providing critical recommendations to primary care providers based on patient needs.

• Delivering brief, targeted therapeutic interventions for clients at various stages of mental health concerns while following the guidelines established by both the Agency and Federally Qualified Health Center (FQHC).
\n
• Collaborating with primary care providers to identify which patients require mental health services within the clinic and which should be referred to specialty care.
\n
• Maintaining regular follow-up communications with patients to monitor their progress, ensuring adjustments to treatment plans are made when necessary.
\n
• Developing personalized relapse prevention plans and interventions for patients at risk, ensuring they are equipped with strategies to maintain stability.
\n
• Diligently monitoring and coordinating the delivery of health services, documenting progress and any critical diagnostic information in the patient’s treatment chart.
\n
• Educating patients on self-management techniques and empowering them to take an active role in their treatment journeys.

Qualifications\n

To be considered for this role, you must possess the following qualifications:

• A Master’s Degree in Clinical Counseling.

• A current Licensed Professional Counselor (LPC) license in the state of Texas.

Preferred Experience\n

While not required, the following skills and experiences are highly desirable:

Compensation and Benefits\n
