Key Responsibilities\n

• Deliver empathetic counseling to both patients and their families, addressing crucial adjustment issues and promoting emotional well-being through evidence-based therapeutic techniques.

• Work collaboratively with a multidisciplinary rehabilitation team to create individualized treatment plans that address the unique psychosocial needs of each patient.

• Guide patients in accessing essential community resources, such as vocational rehabilitation services, housing assistance, and support groups to enhance their recovery process.

• Educate patients and their families on available community resources and the continuum of care post-rehabilitation, ensuring a smooth transition during discharge planning.

• Actively participate in discharge planning, facilitating seamless transitions for patients moving to home care or alternative settings, and ensuring continuity of care.
