Key Responsibilities\n

• Conduct comprehensive assessments to evaluate and address the mental health needs of patients, ensuring tailored treatment plans.

• Foster open communication with supervisors, direct care team members, family members, and other relevant contacts to provide updates on individual progress.

• Maintain a structured schedule for appointments, home visits, and observations to track the advancement of treatment plans.

• Coordinate in-home evaluations and chart reviews while facilitating effective staff interviews to enrich data analysis.

• Monitor the implementation of therapeutic programs and ensure that they align with the individualized treatment plans.

• Engage actively in interdisciplinary team meetings to collaborate with colleagues in the advancement of patient care.

• Document progress notes monthly and quarterly for ongoing evaluation of patient outcomes and adherence to treatment goals.

• Organize training sessions for staff to ensure proficiency in implementing programs and completing relevant documentation.

• Provide continual support through follow-up interviews and home visits to address evolving client needs.

• Maintain organized and detailed records to ensure transparency and accessibility of patient information for all stakeholders.

Qualifications\n
