Key Responsibilities

• Conduct detailed evaluations by reviewing patient histories and performing specific assessments to identify needs and develop clear, tailored treatment plans.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, including goal-setting that involves patient input.
• Implement treatment protocols based on rigorous assessment findings, patient presentations, and best practices in the evolving healthcare landscape.
• Collaborate with fellow healthcare professionals to ensure comprehensive patient care; recognize and respond to changes in patient conditions, adjusting treatment plans as needed.
• Manage discharge planning and follow-up care to facilitate ongoing recovery, documenting all care activities accurately to maintain transparency and continuity.
• Demonstrate excellent time management, meet productivity expectations, and actively participate in marketing initiatives to promote community awareness of our services.
• Contribute to department objectives through initiative, ensuring compliance with all health regulations and quality standards.
• Pursue continuing education opportunities to enhance professional skills and participate as a clinical instructor for students in training.

Patient Population / Client Focus
