Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing medical histories and performing detailed assessments to determine the appropriate treatment strategies.
• Implement individualized treatment protocols that are specifically tailored to each patient's condition, utilizing a variety of therapeutic techniques and modalities.
• Regularly document patient progress, including baseline evaluation, ongoing therapy notes, interim progress summaries, and discharge evaluations, ensuring compliance with regulatory standards.
• Collaborate with other healthcare professionals to discuss patient cases and adjust treatment plans based on clinical findings and patient feedback.
• Provide patient education regarding exercises, rehabilitation techniques, and self-care strategies to empower them in their recovery.
• Engage in patient discharge planning by developing home exercise programs and scheduling follow-up visits, ensuring seamless transitions to independence.
• Participate in ongoing facility meetings and training sessions, maintaining compliance with all corporate and clinical policies.
• Foster respectful communication with patients, their families, coworkers, and external partners to ensure a cohesive care experience.

Patient Population / Client Focus

As a Physical Therapist at Swift Therapy Center, you will primarily work with individuals recovering from orthopedic surgeries, sports injuries, and chronic musculoskeletal conditions. Qualifications

Compensation and Benefits
