Key Responsibilities\n

In your role as a Mental Health Counselor, you will:

• Conduct comprehensive ongoing assessments to evaluate and address the needs of patients, ensuring personalized treatment plans.

• Facilitate communication among supervisors, family members, guardians, and relevant stakeholders to discuss the progress of each individual.
\n
• Provide supervisors with a tentative schedule for appointments, home visits, and observation efforts to monitor client progress and program effectiveness.

• Coordinate home visits that align with in-home observational requirements, engaging in chart reviews and interviews that inform patient-centered strategies.
\n
• Implement and oversee adherence to established program plans while participating actively in Interdisciplinary Team Meetings.
\n
• Document progress through monthly and quarterly notes, ensuring accurate and up-to-date record keeping.
\n
• Organize and conduct training for staff and team members, ensuring competence in implementing program plans and maintaining proper documentation.
\n
• Facilitate follow-up interviews, home visits, and ongoing observations to support continued growth and progress for clients.
\n
• Act as a liaison within the public and mental health communities, promoting collaboration and visibility of developmental goals.

• Contribute to the development of training materials and procedural guidelines that enhance program delivery.

Patient Population / Client Focus\n

Your counseling will reach a broad and diverse client population, including both adults and youth. You will engage with individuals facing varying challenges that may include mental health conditions, family dynamics, trauma, and substance misuse. Qualifications\n

The ideal candidate will possess the following qualifications:
