Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to identify the mental health needs of clients.

• Facilitate communication between clients, their families, and relevant stakeholders to ensure holistic care and support for individual progress.
\n
• Design and provide a tentative schedule of appointments and home visits, monitoring and adjusting as necessary to align with clients’ specific needs.
\n
• Coordinate and execute home visits for in-home observations, facilitating chart reviews and staff interviews to enhance care quality.
\n
• Ensure that the implementation of each client's program plan is effective and carries through as intended to achieve desired outcomes.
\n
• Participate actively in Interdisciplinary Team Meetings, engaging with other professionals to discuss treatment plans and develop comprehensive support strategies for clients.
\n
• Maintain timely and accurate documentation, including monthly and quarterly progress notes to monitor client advancement and support accountability.
\n
• Organize and conduct training sessions to ensure staff are proficient in the procedures required for implementing the program plan and proper documentation.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to reinforce the continued success of client care.

• Serve as a liaison with the public and mental health community to foster collaborative relationships that promote client development goals.

• Assist in refining training materials and develop standard operating procedures to enhance program offerings for clients.

Patient Population / Client Focus\n

Your role will encompass a diverse range of client populations, including both adults and youth. You will utilize your expertise to provide tailored counseling services addressing various mental health issues, including trauma, substance misuse, and family dynamics. Qualifications\n

• An accredited Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Possession of an active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.

• A minimum of two years’ experience in a counseling role, ideally within a clinical context.

• Demonstrated expertise in therapeutic modalities such as family therapy, couples therapy, trauma informed care, or substance misuse treatment.

• A solid understanding of legal and ethical standards governing mental health practices.
\n
• Exceptional interpersonal and communication skills, with a proven ability to establish rapport with a diverse clientele.

• A collaborative approach to working within a multidisciplinary healthcare team.

Preferred Experience\n

Familiarity with electronic medical records (EMR) software and related technologies is highly desirable, as efficient documentation and record-keeping are critical in managing client caseloads effectively.
