Key Responsibilities\n

• Conduct comprehensive ongoing assessments to evaluate the needs and progress of patients, tailoring therapeutic interventions accordingly.

• Communicate effectively with supervisors, direct care staff, family members, and guardians, ensuring everyone is informed of an individual's therapeutic journey.

• Create and manage a tentative schedule that accommodates appointments, home visits, and observations, facilitating efficient tracking of patient progress.

• Cooperate in coordinating home visits for in-home assessments and to gather data for ongoing case management.

• Oversee the implementation of program plans, ensuring they are executed effectively and adjusted as necessary based on patient needs.

• Actively participate in Interdisciplinary Team Meetings to enhance integrated care delivery and share insights on patient outcomes.

• Document monthly and quarterly progress notes comprehensively to maintain accurate and actionable patient records.

• Organize and lead training sessions to certify team members in program implementation and documentation procedures.

• Conduct follow-up interviews and home visits to provide ongoing support and assess the efficacy of treatment plans.

• Maintain meticulous records and documentation to guarantee accessibility of information for collaboration with third parties.

• Build partnerships with the public and the mental health community, providing guidance and support for the development of community mental health initiatives.

• Collaborate in the creation of training materials and methodologies aimed at enhancing support services for individuals.

Patient Population / Client Focus\n

The primary client focus includes a diverse range of populations, from children and adolescents to adults, facing various mental health challenges such as trauma, substance misuse, and family issues. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.

• Minimum of two years of clinical counseling experience, preferably in a direct service setting.

• A strong grasp of ethical and legal standards applicable to mental health care is essential.

• Exceptional interpersonal and communication skills, enabling you to establish rapport with clients from varied backgrounds.
