Hospice RN

We seek a highly skilled and experienced Registered Nurse to provide compassionate, intensive medical end-of-life care in a hospice in-patient unit setting. The nurse will deliver skilled nursing care, symptom management, and support to patients who can no longer be cared for at home, focusing on physical, emotional, social, and spiritual needs. This role requires strong clinical skills, critical thinking, and the ability to educate patients and caregivers. The nurse will collaborate closely with the interdisciplinary team to ensure personalized and quality care. Responsibilities:

• Provide skilled nursing care and symptom management for hospice patients in an in-patient unit.
• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and their families to help them identify personal goals of care and build caregiver confidence.
• Collaborate with interdisciplinary team members to coordinate holistic care addressing physical, emotional, social, and spiritual needs.
• Support patients and families during end-of-life transitions with compassion and professionalism.
• Maintain accurate and timely documentation of patient care and clinical observations.
• Participate in orientation and ongoing training to maintain clinical competencies.

• Active Registered Nurse license in the state of Colorado.
• Minimum of 2 years nursing experience in hospital, home health, or hospice care.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Excellent communication and interpersonal skills.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for field work.

• Experience in hospice or palliative care nursing.
• Advanced symptom management and end-of-life care expertise.
• Ability to work effectively in a fast-paced, emotionally demanding environment.

• On-call requirement of 2 shifts per month.
• Orientation includes 3 days from 8am to 5pm before transitioning to night shifts.
• Work environment involves providing care in a home-like hospice facility supporting patients and families during difficult times.

• Full-time position with night shifts following orientation.
• Shift assignments include on-call responsibilities as specified.
