Work Environment

This role requires working full-time onsite within the patient's home, which provides a unique chance to build strong relationships with patients in their own environments. You will regularly engage with patients, their families, and healthcare professionals to deliver coordinated and comprehensive care. Expect an average work schedule of 40 hours per week, from 8:00 a.m. to 5:00 p.m., allowing you to actively participate in shaping your patient’s healthcare journey.

Key Responsibilities

• Perform thorough health assessments, including in-depth medical history collection, physical examinations, and routine health screenings to establish baseline health metrics.
• Diagnose and manage both acute and chronic health conditions, ensuring that patients receive appropriate treatment and interventions timely.
• Provide essential education on disease prevention and health maintenance, empowering patients and their families to make informed health decisions.
• Develop personalized care plans that reflect the unique health needs and preferences of each patient, adjusting as necessary to enhance outcomes.
• Coordinate care with multidisciplinary teams, including primary care physicians and specialists, to ensure continuity of treatment.
• Utilize electronic health records (EHR) systems for accurate documentation and to facilitate communication with other healthcare providers while respecting patient confidentiality.
• Engage in patient follow-up visits to monitor progress and adapt care plans accordingly, enhancing patient engagement and satisfaction.
• Stay current with best practices in primary care and home healthcare, incorporating new knowledge into your everyday practice.

Patient Population / Client Focus

In your role, you will primarily work with a diverse patient population, including individuals with chronic illnesses, post-operative care needs, and those requiring preventive care. Many patients may experience mobility challenges or other barriers to receiving traditional care, making your role in educating and supporting them in their home setting crucial for promoting adherence and improving their overall health.

Qualifications

• Valid Nurse Practitioner certification and an active nursing license in the state of Texas.
• A Bachelor’s Degree in Nursing or a related field, with a strong emphasis on patient-centered care.
• A minimum of two years of clinical experience in a primary care or similar setting.
• Exceptional communication skills, both verbal and written, to effectively interact with patients and collaborate with multidisciplinary teams.
• Strong organizational skills to manage multiple patient care plans and documentation responsibilities efficiently.
• Ability to work independently and be self-motivated, demonstrating a commitment to improving patient care.

Preferred Experience

• Experience in home care settings or similar environments is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth services is a plus.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 to 220,000 annually, commensurate with experience and qualifications.
