Work Environment

Working onsite at our assisted living facility, you will be part of a dynamic and collaborative healthcare team. Your typical work hours will be from 8:00 a.m. to 5:00 p.m., providing a structured 40-hour work week to allow for patient-focused practices without excessive demands on your time.

Key Responsibilities

• Conduct thorough assessments to evaluate patients' health statuses, including physical exams and medical histories.
• Diagnose both acute and chronic medical conditions, utilizing evidence-based guidelines.
• Develop and implement individualized treatment plans tailored to the diverse needs of each resident.
• Prescribe medications and therapies as necessary, ensuring compliance with regulations and safety protocols.
• Order and interpret diagnostic tests, leveraging findings to adjust treatment strategies.
• Provide patient education to promote disease prevention, health maintenance, and necessary lifestyle modifications.
• Collaborate with interdisciplinary teams, including physicians, nurses, and social workers, to coordinate comprehensive patient care and referrals.
• Maintain accurate electronic health records (EHR) and ensure timely documentation for all patient interactions.
• Engage in quality improvement initiatives, advocating for adherence to clinical guidelines and best practices.

Patient Population / Client Focus

The primary focus of this role is the adult population residing in the assisted living facility. You will address a wide range of health issues, from chronic diseases like diabetes and hypertension to acute conditions that require immediate attention. Building trusting relationships with residents is vital, as many individuals seek continuous care and support throughout their living experience.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.
• A master's degree in nursing from an accredited institution.
• A minimum of 3 years of clinical experience in primary care, preferably in a geriatric or assisted living context.
• Strong communication skills, enabling effective interaction with patients, families, and healthcare teams.
• Proficient analytical and problem-solving abilities to address complex patient needs.

Preferred Experience

• Experience working within an assisted living or long-term care environment is highly desirable.

Compensation and Benefits

This position offers a competitive salary range of USD 120,000 to 220,000 per year, commensurate with relevant experience and qualifications.
