Key Responsibilities

• Conduct comprehensive evaluations, including the review of medical history and performance of specific assessments to establish individualized treatment plans.
• Communicate evaluation findings and treatment plans effectively with patients and their families, actively involving them in goal-setting processes to foster a collaborative approach to care.
• Implement tailored treatment protocols based on evaluations, with adjustments made as necessary to respond to the evolving health care landscape and patient progress.
• Consult with interdisciplinary healthcare professionals regarding patient care and document any significant changes in patient condition requiring modifications to the plan of care.
• Execute thorough discharge planning and follow-up care, ensuring accurate and timely documentation of patient progress and treatment outcomes throughout their rehabilitation journey.
• Demonstrate strong time management skills by maintaining an accurate schedule and adhering to productivity expectations, while actively participating in community outreach and hospital marketing activities.
• Contribute to departmental goals through collaboration and compliance with governmental and accreditation standards.
• Pursue continuous education for your professional growth and lead clinical education for students, enhancing the learning environment within our clinic.

Patient Population / Client Focus

As a Physical Therapist at our clinic, you will primarily work with a diverse patient population, including individuals recovering from surgeries, managing chronic conditions, or seeking rehabilitation for injuries. Qualifications

• Possession of a Doctorate degree in Physical Therapy (DPT) is required.
