RN Geriatric Care

Job Summary:

The Registered Nurse (Level 2) will provide skilled nursing care to geriatric residents in a long-term care setting, managing up to 20 residents per shift. The role includes direct patient care, assessments, documentation, medication administration, and oversight of LPN and CNA staff. The nurse will collaborate closely with the care team to implement individualized nursing care plans, focusing on wound care and chronic condition management without acute care needs. Strong communication, delegation, and flexibility are essential for success in this role. Responsibilities:

• Provide direct nursing care and assessments for long-term care residents.
• Administer medications and treatments as prescribed.
• Document patient status, nursing interventions, and care plans accurately.
• Oversee and delegate tasks to LPNs and CNAs assigned to the unit.
• Collaborate with interdisciplinary team members to coordinate resident care.
• Manage wound care and monitor residents' chronic health conditions.
• Ensure compliance with facility policies and regulatory standards.
• Maintain effective communication with residents, families, and healthcare providers.

Required Skills & Certifications:

• Active New York State Registered Nurse (RN) license.
• Minimum of two years' experience in a long-term care nursing environment.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong clinical assessment, medication administration, and documentation skills.
• Ability to supervise and collaborate with licensed and unlicensed nursing staff.
• Excellent communication and interpersonal skills.

Preferred Skills & Certifications:

• Experience with telemetry monitoring.
• Prior experience in geriatric or wound care nursing.
• Familiarity with electronic health record systems such as EPIC.

Special Considerations:

• Work includes day, evening, and night shifts with weekend, holiday, and on-call coverage as needed.
• Long-term care environment with no acute care or tracheostomy patients.
• Must be able to provide care for older adults with chronic health conditions and wounds.

• Shifts available include days, evenings, and nights.
• Scheduling includes weekends, holidays, and on-call duties based on unit needs.
