Key Responsibilities\n

• Facilitate crisis intervention and develop pre-crisis and crisis response strategies, implementing evidence-based methodologies to manage critical situations effectively.

• Utilize online documentation systems to ensure all clinical notes are completed in accordance with regulatory standards and best practices.

• Collaborate with multidisciplinary teams by attending case conferences and actively participating in the formulation and execution of positive behavior support plans.

• Maintain productive relationships with other service providers, state agencies, and community partners to enhance resource availability for your clients.

• Regularly engage in professional development opportunities such as workshops and conferences to stay current on best practices within the field of social work.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse clientele comprising individuals and families who may be struggling with issues such as depression, anxiety, substance abuse, and experiences related to trauma or abuse. By proactively addressing these challenges, you will help clients access available resources while promoting resilience and recovery.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited academic institution is essential.

• Active state licensure in good standing (LCSW) for the state of Colorado.

• At least two years of supervised clinical experience is required to ensure a well-rounded understanding of the complexities facing patients in a community health setting.
