Key Responsibilities\n

As a Licensed Professional Counselor at Frontier, your responsibilities will encompass:

• Conducting thorough patient intakes that include diagnostic and functional evaluations, leading to tailored treatment recommendations for primary care providers.

• Offering brief, focused therapeutic interventions designed to address immediate mental health concerns.
\n
• Collaborating closely with primary care teams to identify which patients require specialty mental health services versus those who can be managed within primary care.

• Following up with patients to assess their progress and adjust treatment strategies accordingly.

• Developing and maintaining relapse prevention plans to support patients in achieving and sustaining mental wellness.
\n
• Monitoring behavioral health service delivery and ensuring coordination of care with external providers when necessary.
\n
• Documenting patient progress comprehensively in clinical records, while keeping primary care providers informed of any changes in patient status.

• Providing valuable self-management skills and educational resources to empower patients during their recovery journey.

• Consulting with clinic management regarding behavioral health outcomes and program evaluations.

• Assisting patients in complying with primary care treatment plans, which may include offering strategies for managing medication side effects.

Qualifications\n

To be considered for this position, candidates must meet the following qualifications:

Compensation and Benefits\n

The salary range for this position is competitive, with an annual salary between USD 87,000 and 105,000 .
