Key Responsibilities\n

• Provide clinical consultation to our diverse clinical staff regarding complex mental health issues, including those affecting geriatric populations and individuals with substance use disorders.

• Interpret and relay critical patient information, including social histories, to staff and other healthcare providers, ensuring a coordinated approach to care.
\n
• Develop and implement individualized patient care plans based on thorough assessments, coordinating recommendations particularly focused on the home environment and available support systems.

• Conduct treatment planning meetings, reviewing and discussing cases with providers to implement best practices and achieve optimal patient outcomes.

• Maintain accurate and timely progress notes after client sessions to ensure compliance with regulatory standards and improve continuity of care.

• Perform additional related duties as assigned to support the clinical operations and enhance patient care.

Patient Population / Client Focus\n

Our counselors work closely with a variety of client populations, including elderly individuals navigating age-related challenges, patients with severe mental health conditions, and those overcoming substance use issues. We prioritize culturally competent care, understanding the unique needs of clients from diverse backgrounds and ensuring their voices are heard in treatment planning.

Qualifications\n

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Possession of a current and independent license is essential; candidates must hold one of the following: LMHC (Licensed Mental Health Counselor), LCSW (Licensed Clinical Social Worker), or LMFT (Licensed Marriage and Family Therapist).

• Strong proficiency in writing detailed and coherent progress notes following client sessions along with a proven ability to manage a caseload effectively.

Preferred Experience\n

While all qualified applicants are encouraged to apply, preference will be given to those with hands-on experience in telehealth services and a background working with geriatric populations or clients dealing with addiction-related issues. Familiarity with electronic health record (EHR) systems and telehealth platforms can further enhance candidate appeal.

Compensation and Benefits\n
