Key Responsibilities\n

As an LCPC at Cure Connect Consulting, your daily responsibilities will include but are not limited to:

• Conducting ongoing assessments of patients to understand their unique needs and establish personalized treatment plans.
\n
• Maintaining open lines of communication with supervisors, healthcare providers, family members, and guardians regarding each patient’s progress.
\n
• Providing a tentative schedule for appointments, home visits, and necessary observations to effectively track and support patient development.
\n
• Coordinating home visits to ensure thorough monitoring, which includes chart reviews and data analysis to refine treatment approaches.
\n
• Ensuring that the program plan is implemented efficiently and adjusts to meet patients' evolving needs.
\n
• Participating in Interdisciplinary Team Meetings to collaborate on effective treatment strategies and share insights.
\n
• Completing monthly and quarterly progress notes to document patient advancements and inform future care directions.
\n
• Organizing staff training and professional development sessions to ensure team members fully understand program implementation and documentation.
\n
• Conducting follow-up interviews, home visits, and sessions to provide consistent support tailored to each individual's requirements.
\n
• Maintaining accurate records and documentation to ensure transparency and accessibility of information for all stakeholders.
\n
• Acting as a liaison with the public and mental health community to contribute towards development goals and enhance community outreach.
\n
• Assisting in the creation of training materials to enhance program workflows for ongoing improvement.

Patient Population / Client Focus\n

Your work will involve a diverse range of clients, including adults and youth, each facing unique challenges in their mental health journeys. You will work with individuals dealing with issues such as anxiety, trauma, depression, family conflicts, and substance misuse, employing evidence-based practices to promote their recovery. Your approach will be tailored, respectful, and anchored in both compassion and professionalism.

Qualifications\n

To be successful in this role, you are required to possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.
\n
• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
\n
• A minimum of two years of counseling experience, preferably in a clinical setting.

• Deep understanding of legal and ethical standards pertinent to mental health counseling.
\n
• Strong interpersonal skills, with the ability to build rapport and trust with varied clientele.

• Demonstrated collaboration with multidisciplinary healthcare teams.
\n
• Proficiency with Electronic Medical Records (EMR) software and adeptness in using modern healthcare technologies.

Compensation and Benefits\n
