Key Responsibilities\n

• Provide individualized psychotherapy and mental health services to patients, utilizing various therapeutic modalities to assist those experiencing anxiety, depression, PTSD, and other mental health conditions.

• Conduct comprehensive assessments to evaluate patients' mental health status and their emotional, behavioral, and social functioning.

• Develop and implement tailored, evidence-based treatment plans that are culturally competent and address the specific needs of each patient, incorporating patient preferences and feedback.

• Utilize electronic health records (EHRs) to manage patient information efficiently, ensuring effective coordination of care and documentation of progress in treatment.
\n
• Collaborate with a multidisciplinary team of healthcare professionals to optimize patient outcomes, sharing insights and strategies that enhance the overall treatment process.

Patient Population / Client Focus\n

This role primarily serves patients in a hospice care setting, many of whom face terminal illnesses alongside mental health challenges. You will work closely with patients and their families to navigate emotional difficulties, providing support and counseling during what can be a profoundly challenging time. The ideal candidate will approach each interaction with sensitivity and understanding, ensuring that patients feel heard and valued.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
