In our telehealth setting, counselors will play a pivotal role in providing crucial mental health support to various patient populations while working remotely from Mesa, Arizona. Key Responsibilities\n

• Collaborate closely with primary care providers to recognize, address, and manage mental health and psychosocial issues, serving as an integral part of an interdisciplinary patient care team.

• Conduct comprehensive patient assessments, focusing on both diagnostic and functional evaluations. Provide actionable recommendations to primary care providers regarding tailored treatment goals and individual plans for each patient.

• Deliver targeted and effective therapeutic interventions for patients requiring mental health support, ensuring services provided align with established best practices.
\n
• Guide primary care providers in making informed decisions about patient referrals to specialized mental health facilities when necessary, thereby enhancing continuity of care.

• Implement follow-up procedures to monitor patient progression and determine needed adjustments in treatment methodologies, ensuring a responsive approach to mental health management.

• Develop comprehensive relapse prevention plans and support patients in maintaining stable functioning through verbal encouragement and practical strategies.
\n
• Identify at-risk patients and design proactive plans to decrease the likelihood of deteriorating mental health conditions.

• Oversee and coordinate behavioral health services, fostering connections both within the clinic and with external resources, after obtaining patient consent.

• Document and maintain accurate records of patient progress and diagnostic information to uphold continuity in treatment.
\n
• Engage in continuous communication with primary care providers to share insights on patient needs and progress, collaborating to optimize treatment strategies.

• Assist patients in adopting healthier lifestyle choices and coping strategies that facilitate positive behavioral changes throughout their recovery journeys.

• Provide educational materials and self-management skills to empower patients and support their active roles in their treatment.
\n
• Consult with clinic management and fellow team members on behavioral services, proposing evaluation areas for program enhancement and effectiveness.

• Support patient adherence to prescribed medical treatments, including strategies for managing medication side effects.

• Adhere strictly to ethical guidelines and the Code of Ethical Conduct for Clinical Professionals.

• Execute any additional assigned duties that contribute to the clinic’s mission and patient care goals.

Qualifications\n

• A Master’s Degree in Clinical Counseling is mandatory.

• Must possess a current Licensed Professional Counselor (LPC) License and meet all local and state regulatory requirements.

• Demonstrable knowledge in mental health assessment, treatment planning, developmental psychology, and a variety of therapeutic modalities including family therapy, group therapy, and behavioral therapy.

• Proficiency in English is required; bi-lingual capabilities, especially in Spanish, are preferred.

Compensation and Benefits\n

This position offers a competitive salary of USD 87,000 – 105,000 annually, commensurate with experience and qualifications. Our clinic provides a comprehensive benefits package that includes medical, dental, and vision insurance, generous paid time off (PTO), a retirement plan, and professional development opportunities. As an added incentive, a sign-on bonus will be awarded to newly hired counselors.
