Key Responsibilities\n

• Engage in patient intakes that include comprehensive diagnostic and functional evaluations, recommending individualized treatment goals to primary care providers.

• Deliver brief, focused therapeutic interventions for patients, assisting with immediate mental health concerns.
\n
• Collaborate with primary care providers to assess which patients can receive adequate care within the primary setting and identify those requiring specialized mental health interventions.
\n
• Implement follow-up strategies to monitor patient progress, making necessary adjustments to care plans as needed.
\n
• Develop relapse prevention strategies and promote stability among patients experiencing mental health challenges.
\n
• Identify patients at risk of deterioration and devise actionable prevention plans that enhance long-term mental health outcomes.
\n
• Document patient progress and therapeutic interventions meticulously within treatment charts to ensure continuity of care.
\n
• Facilitate communication with primary care providers to keep them informed of patient needs and progress, assisting in the formulation of treatment plans.
\n
• Provide education and resources to empower patients in self-management and recovery, ensuring they are active participants in their treatment journey.
\n
• Consult with clinic management and interdisciplinary team members regarding behavioral health services and potential program evaluations.

• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals throughout your practice.

Patient Population / Client Focus\n

Your role primarily involves working with a diverse patient population including families, individuals facing substance abuse issues, children, and adolescents with behavioral challenges. You will address a wide range of mental health issues, custom-tailoring interventions to suit each patient’s needs and circumstances, thereby making significant contributions to the overall health and resilience of the community.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling is required.

• Must possess a current LPC (Licensed Professional Counselor) license.
\n
• In-depth knowledge of mental health assessment, treatment planning, and various therapeutic modalities including cognitive-behavioral therapy and family systems therapy.

• Ability to evaluate and understand the social and psychological factors affecting a patient’s life.
\n
• Proficient in maintaining accurate clinical records and reporting.
\n
• Capacity to collaborate effectively with a multidisciplinary team and establish strong relationships with patients and their families.

Preferred Experience\n

• Fluency in both English and Spanish is highly desirable, enhancing communication with our diverse patient base.

• Experience in community mental health settings or Federally Qualified Health Centers (FQHC) is an advantage.

• Demonstrated ability to guide patients in making healthier lifestyle choices and behavioral changes that contribute to their well-being.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications. Additionally, new hires will receive a sign-on bonus to welcome them to our team.
