Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments to understand clients' needs and develop personalized treatment plans that adhere to best practices.

• Implement and document evidence-based interventions to promote clients' emotional well-being and support their families throughout the hospice journey.

• Maintain meticulous and confidential clinical records, ensuring compliance with both state and federal regulations.

• Collaborate closely with a multi-disciplinary treatment team, including medical providers, nurses, and other healthcare professionals, to coordinate comprehensive care.

• Educate clients and their families about the hospice process and available resources, providing ongoing emotional support and crisis intervention as needed.

• Stay informed on the latest research, advancements, and techniques in mental health and hospice care to enhance service delivery.

Patient Population / Client Focus\n

In this role, you will work with individuals who are nearing the end of life and their families, helping them navigate emotional challenges and existential questions that arise during this time. Qualifications\n
