Work Environment\n

This position is a remote role, allowing you the flexibility to manage your time effectively while remaining connected to a supportive team of healthcare professionals. The work schedule is designed for work-life balance with hours from 9 AM to 6 PM, Monday through Friday, with no evening or weekend work required. This structure promotes a healthy work environment and allows our staff to focus on their roles without the stresses associated with after-hours commitments.

Key Responsibilities\n

• Conduct comprehensive assessments of mental health needs, followed by detailed treatment planning tailored to individual patient requirements.

• Manage implementation of behavioral health services, coordinating efforts with a multidisciplinary primary care team.

• Evaluate and conduct intake assessments for new clients, ensuring timely and respectful service delivery.
\n
• Provide counseling and crisis intervention as necessary, employing evidence-based practices to address immediate client needs.
\n
• Facilitate referrals for patients requiring inpatient or specialty care, connecting them with appropriate resources outside of the hospice center.
\n
• Screen clients for substance use disorders, including prescription drug abuse and alcoholism, providing supportive counseling and referrals as needed.
\n
• Regularly communicate and collaborate with other healthcare providers to deliver integrated care and uphold high standards of service.

• Maintain adherence to all mental health policies and procedures throughout the behavioral health program, ensuring compliance with local, state, and federal regulations.

Patient Population / Client Focus\n

In your role as LCSW, you will work primarily with clients dealing with varied mental health challenges within a hospice framework. Patients may be experiencing emotional distress related to terminal illness, end-of-life issues, and family dynamics associated with caregiving and grief. Your work will not only focus on alleviating symptoms but also on enhancing the overall quality of life for patients and their loved ones during critical moments.

Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited institution.

• A current and active license to practice as a Licensed Clinical Social Worker (LCSW).
