Key Responsibilities\n

• Conduct Comprehensive Assessments: Perform detailed evaluations of clients to identify their specific mental health needs and formulate individualized treatment plans.

• Interdisciplinary Communication: Maintain open lines of communication with supervisors, healthcare staff, families, and other stakeholders regarding client progress and care coordination.

• Implementation and Monitoring: Oversee the execution of treatment plans to ensure they are being followed accurately and effectively, with regular updates to supervisors.

• Home Visits and Observations: Coordinate and conduct necessary home visits and observations to gather detailed insights into client circumstances and progress.
\n
• Documentation: Write comprehensive monthly and quarterly progress notes and ensure records are up to date, organized, and accessible to relevant parties.
\n
• Team Collaboration: Actively participate in team meetings, sharing insights and contributing to case discussions to optimize client outcomes.
\n
• Training and Development: Lead training sessions for staff on the implementation of treatment protocols and best practices in documentation to ensure consistency and efficacy in service delivery.

• Liaison Role: Act as a bridge between clients and the mental health community, advocating for resources and support to help clients achieve their developmental goals.

Patient Population / Client Focus\n

This position involves working with a diverse client base, including adults and youth, presenting a variety of mental health challenges. The LPC will be responsible for tailoring therapeutic approaches to meet the unique needs of individuals and families, including those facing issues such as trauma, substance misuse, and relationship difficulties.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active, unrestricted licensure as a Professional Counselor in the state of New Jersey.

• Minimum of 2 years of experience in counseling, preferably in a clinical context.
