Work Environment

Key Responsibilities

• Conduct thorough initial assessments to evaluate patients' functional limitations and disabilities, leading to the development of tailored treatment plans.
• Implement and modify treatment plans based on ongoing assessments and patient progress, utilizing evidence-based practices to achieve optimal outcomes.
• Provide education to patients and their families regarding diagnoses, treatment choices, and strategies to facilitate recovery.
• Maintain clear and consistent documentation of evaluations, treatment progress, and outcomes according to applicable laws and standards.
• Collaborate with a multidisciplinary team, including physicians and other healthcare professionals, to ensure comprehensive patient care and smooth transitions throughout the continuum of care.
• Participate in leadership activities such as community outreach initiatives aimed at raising awareness about physical therapy services and health promotion.
• Engage in ongoing professional development through participation in educational programs and staying updated with the latest treatment modalities.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work with a diverse patient population, including individuals recovering from surgery, chronic pain patients, and those needing rehabilitation for age-related conditions. Your empathetic approach will be key in addressing their unique needs and challenges in a home setting.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program required.
• Current state license in Physical Therapy and BLS certification are mandatory.
• Excellent communication skills, both written and verbal, with proficiency in Microsoft Office and documentation software.

Preferred Experience

• A minimum of two years of experience as a Physical Therapist, preferably in a home care or outpatient setting.
• Experience in developing patient-centered treatment plans and outcomes measurement.

Compensation and Benefits
