Key Responsibilities\n

• Provide direct clinical services to clients experiencing symptoms of major mental illnesses or severe emotional disorders. This includes facilitating individual, group, and family therapy sessions.

• Engage in the assessment process, conducting brief, evidence-based screenings and arranging for more comprehensive assessments as warranted.

• Develop, implement, and monitor integrated care plans, ensuring seamless access to linked services and fostering communication between clients, families, and providers.

• Participate as an active team member in care coordination and crisis intervention efforts, providing timely support during emergency situations.
\n
• Maintain compliance with ethical guidelines and organizational policies, ensuring a high standard of care and professional integrity.
\n
• Monitor client progress and outcomes through regular evaluations while ensuring that treatment services align with client-defined goals.
\n
• Contribute to quality improvement initiatives and participate in committees aimed at enhancing service delivery and operational efficiency.

• Ensure the timely completion of all required documentation, case notes, and data submissions in accordance with departmental protocols.

• Provide on-call coverage as needed during and after program hours to support client needs.

• Identify and report unsafe situations, actively engaging in procedures designed to maintain a safe therapeutic environment.

Qualifications\n

• A Master’s Degree in Social Work (MSW) or a related field is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut is mandatory.

• At least 2 years of direct clinical experience working in a mental health setting or with a similar population is strongly preferred.

• In-depth knowledge of assessment, diagnosis, and treatment planning, including crisis management and risk assessments.

• Exceptional verbal and written communication skills, with an emphasis on building rapport with clients and collaborating with interdisciplinary teams.
