Key Responsibilities\n

As a Licensed Professional Counselor with Reliant Health Center, you will be responsible for the following:

• Conduct regular and thorough assessments to understand the needs of your patients and tailor treatment plans accordingly.

• Communicate effectively with supervisors, direct care staff, family members, and guardians about patient progress and treatment updates.
\n
• Manage a schedule of appointments, home visits, and observations, coordinating efforts to ensure effective monitoring of treatment outcomes.

• Conduct home visits to perform in-depth observations, chart reviews, and interviews to assess individual needs and progress.
\n
• Implement and review program plans, ensuring that each element is enacted effectively and aligns with established treatment goals.
\n
• Participate actively in interdisciplinary team meetings to share insights and collaborate on treatment strategies.
\n
• Maintain clear and comprehensive monthly and quarterly progress notes that are aligned with patient treatment plans.
\n
• Organize and lead training sessions to ensure staff competence in program execution and documentation standards.
\n
• Provide ongoing follow-ups, including interviews and additional home visits, to assess continued support requirements for patients.

• Keep precise and organized records to facilitate easy access to patient information for all stakeholders involved.

• Act as a liaison with the community and mental health organizations to foster support for local development initiatives.

• Assist in the creation of training materials that enhance the effectiveness of programs designed for the improvement of client services.

Patient Population / Client Focus\n

Your work will encompass a diverse client base, including both adults and youth dealing with various mental health challenges. Qualifications\n

The ideal candidate will possess:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• A valid and unrestricted license as a Professional Counselor (LPC) in the state of Oregon.

• A minimum of two years of counseling experience, preferably within a clinical setting.

Preferred Experience\n
