Key Responsibilities\n

• Provide compassionate psychotherapy and mental health services to patients facing various mental health conditions, including anxiety, depression, and end-of-life challenges.

• Conduct thorough and empathetic assessments of patients’ mental health status, focusing on emotional, behavioral, and social functioning to create a tailored treatment approach.

• Develop, implement, and continuously evaluate evidence-based treatment plans that respect each patient’s unique cultural background and personal circumstances.

• Collaborate closely with interdisciplinary hospice teams to coordinate care effectively and deliver comprehensive support to patients and their families.

• Utilize electronic health records to maintain accurate documentation of patient interactions, treatment plans, and progress notes, ensuring compliance with all relevant legal and ethical standards.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care facing complex emotional and psychological needs related to chronic and terminal illness. Your expertise will be crucial in addressing issues related to grief, loss, and coping, offering your patients comfort and assistance during one of the most challenging times in their lives.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
