Key Responsibilities\n

As a Licensed Professional Counselor at Turning Points Clinic, you will play a vital role in shaping treatment plans and enhancing client experiences. Your responsibilities will include:

• Participating in the planning and organization of outpatient mental health programs, ensuring alignment with best practices and patient needs.

• Communicating regularly with physicians and other healthcare team members to provide updates on patient progress, assess needs, address concerns, and identify potential problems.

• Keeping comprehensive and up-to-date clinical documentation, including progress notes and treatment summaries, in accordance with departmental policies.

• Collaborating closely with your healthcare team to develop individualized treatment plans and discharge strategies based on thorough psychosocial assessments.

• Providing counseling services, including conducting thorough assessments and offering crisis intervention for patients facing emotional and behavioral challenges.

• Engaging with clients' families and support systems to facilitate effective treatment processes and discharge planning.

Your primary focus will be on high-risk patients who often face socio-economic and psychological barriers to care. Qualifications\n

Candidates for this role must meet the following qualifications:
