Work Environment

This position is based onsite at our modern rehabilitation facility, equipped with the latest therapeutic equipment and technologies designed to optimize the recovery process for our patients. You will work Monday through Friday, from 8 AM to 5 PM, within a collaborative team of healthcare professionals committed to fostering a positive atmosphere that encourages professional growth and continuous learning.

Key Responsibilities

As a Physical Therapist at OnTrack Physical Therapy & Wellness, you will:

• Conduct thorough evaluations, including taking detailed patient histories, performing specific assessments such as range of motion and functional tests, and developing comprehensive treatment plans based on patient needs and evaluation findings.
• Implement individualized treatment protocols, ensuring that they align with evidence-based practices and are responsive to the evolving healthcare environment.
• Regularly communicate and collaborate with other healthcare providers, including physicians and specialists, to ensure comprehensive patient care and timely modifications to treatment plans as necessary.
• Document all patient care activities meticulously, including initial evaluations, daily progress notes, and discharge summaries, ensuring compliance with regulatory standards and maintaining a clear and accurate medical record.
• Participate in discharge planning and follow-up care to ensure a smooth transition for patients, providing them with the tools and education necessary to maintain progress post-therapy.
• Maintain open communication channels with team members, patients, and their families, fostering trust and ensuring that all parties are engaged in the recovery process.
• Contribute to facility meetings and continuing education sessions, sharing insights and staying updated on best practices in physical therapy.

Patient Population / Client Focus

You will primarily work with diverse orthopedic populations, including post-operative patients, athletes recovering from injuries, and those with chronic pain conditions. Your tailored approach will be crucial in helping patients regain strength, mobility, and confidence as they navigate their rehabilitation journeys.

Qualifications

• Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy (DPT) is preferred.
• Current physical therapy license in the state of Florida, meeting all local and federal regulations.
• Certification in Basic Life Support (BLS) from the American Heart Association.

Preferred Experience

We recommend candidates to have at least two years of clinical experience in a rehabilitation setting, with proven expertise in treating orthopedic conditions. Experience with electronic health record (EHR) systems and familiarity with patient management software will also be beneficial.

Compensation and Benefits
