Key Responsibilities\n

• Conduct age and culturally appropriate biopsychosocial assessments to evaluate mental health, substance abuse, and medical needs.

• Deliver behavioral health services, which include individual and group counseling, intake assessments, treatment planning, crisis intervention, and referral to substance use treatment programs.

• Document patient interactions meticulously, including mental health evaluations, progress notes, and suicide risk assessments, using electronic health record systems.

• Coordinate with local resources and community agencies to ensure our clients receive comprehensive support, including identifying medication assistance programs.

• Collaborate with Quality Improvement initiatives to track performance and develop data collection strategies to enhance our service delivery.

Qualifications\n

• A current and valid license as a Licensed Clinical Social Worker (LCSW) in the state of New York.
