Work Environment\n

This position offers the flexibility of remote work while focusing on client interactions through virtual appointments and telehealth solutions. As a member of our social work team, you will collaborate with interdisciplinary professionals, including nurses and caregivers, to ensure seamless connectivity and integrated care for our clients. Although remote, you will occasionally need to be available for on-site visits to assess clients in their home environments when necessary.

Key Responsibilities\n

• Conduct detailed psychosocial assessments of seniors to identify emotional, mental, and social needs.

• Develop and implement individualized care plans in collaboration with clients and their families to address specific life goals and challenges.

• Provide ongoing counseling services, assisting clients in managing mental health issues, family dynamics, and life transitions.

• Advocate for clients' access to resources, including healthcare services, support groups, and community programs aimed at enhancing quality of life.

• Organize regular check-ins to monitor client well-being and living conditions, ensuring their safety and comfort.

• Facilitate open communication among family members, promoting conflict resolution in the caregiving process.

• Guide clients through available community resources to foster engagement and social interaction.

• Maintain comprehensive and accurate records of client encounters, assessments, care plans, and updates within electronic health record (EHR) systems.

• Stay updated with best practices, ethical standards, and regulatory requirements within the social work profession.

Patient Population / Client Focus\n

The primary focus of this role is to work with seniors, aged 65 and older, who may be experiencing various challenges related to aging, including mental health concerns, chronic illnesses, and social isolation. You will collaborate closely with family members and caregivers to ensure that seniors receive appropriate interventions to support their mental, emotional, and social well-being.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• Minimum two years of experience in a social work capacity, preferably in geriatric settings or community health.

Compensation and Benefits\n

We offer a competitive annual salary ranging from $60,000 to $100,000, commensurate with experience and expertise.
