Key Responsibilities

• Conduct thorough evaluations of patients recovering from injury, surgery, or illness within a home care and facility setting to determine their current functional capacities.
• Design and implement customized treatment plans that may include therapeutic exercises, educational activities, and manual therapy techniques to enhance mobility and alleviate pain.
• Utilize a variety of therapeutic modalities, including ultrasound and electrical stimulation, as appropriate for individual patient conditions.
• Maintain accurate and detailed documentation of patient treatments, progress, and response to therapies in compliance with state regulations.
• Regularly reassess patient conditions, modifying treatment plans as necessary to ensure that therapeutic goals are achieved.
• Fit and train patients in the proper use of assistive devices, including crutches, walkers, and wheelchairs, ensuring safety and optimal function.
• Provide education to patients, caregivers, and family members regarding home exercise programs, injury prevention strategies, and overall wellness.
• Stay updated on the latest research and advancements in physical therapy practice through continuous professional development and workshops.

Patient Population / Client Focus

You will be working with a diverse patient population, including pediatrics and adults with varying physical needs and diagnoses. This may include orthopedic injuries, post-surgical rehabilitation, neurological conditions, and chronic pain management. Your role will be pivotal in helping patients regain their independence and improve their quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• A state-issued license in Physical Therapy is mandatory to practice.
• Strong communication skills, both verbal and written, are essential for effective patient interaction and collaboration with the health care team.
• Ability to demonstrate flexibility and adaptability in a fast-paced environment.
• Current CPR certification must be maintained throughout employment.
• Basic computer skills, including familiarity with EMR systems for documentation.

Preferred Experience

• While prior experience of two years in a similar role is preferred, new graduates with a Doctorate in Physical Therapy (DPT) are encouraged to apply.
• Experience in a multidisciplinary rehabilitation environment is a plus.

Compensation and Benefits
