Work Environment

This position will be fully onsite, allowing Physical Therapists to work directly in patients' homes, providing tailored care that considers the unique challenges of each individual's living situation. The role encompasses 40 hours of scheduled work per week, with variable weekends as needed to accommodate patient requirements. Physicians, referral sources, and an interdisciplinary treatment team support our therapists, ensuring comprehensive care is delivered to every patient.

Key Responsibilities

• Conduct thorough examinations and evaluations of patients with diverse functional limitations and disabilities to develop effective treatment plans.
• Offer expert consultation to other healthcare providers regarding the continuum of care for each patient, ensuring smooth transitions.
• Engage with patients of all ages and cultural backgrounds, adjusting communication styles to meet individual needs while upholding customer service standards.
• Document evaluation findings, treatment implementation, and progress with precision, adhering to all regulatory standards and organizational protocols.
• Participate in leadership functions, including community outreach initiatives, continuous quality improvement (CQI) efforts, cost containment activities, and program development.
• Supervise assigned support staff, effectively managing departmental resources and training new hires as necessary.
• Maintain open channels of communication with team members, physicians, and patients to optimize care delivery, participating in regular staff meetings and continuing education sessions to enhance skills.

Patient Population / Client Focus

The role primarily focuses on serving individuals with various healthcare needs in the home care setting, including but not limited to elderly patients requiring rehabilitation post-surgery, individuals with chronic pain, and patients recovering from trauma or injury. This diverse clientele necessitates a comprehensive approach to treatment, tailored to each patient's circumstances and preferences.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid state license to practice Physical Therapy is mandatory.
• Effective written and verbal communication skills are essential for interacting with patients and collaborating with the healthcare team.
• Proficiency in word processing and computer skills is necessary for documentation and communication.

Preferred Experience

Candidates with at least two years of experience in a clinical setting are preferred, particularly those with backgrounds in home health care or outpatient therapy environments.
