Work Environment

This position is onsite, enabling you to work directly within the home environments of your clients. You will engage with patients in the Dulles area, delivering therapy in a one-on-one setting that fosters deep, meaningful interaction and assessment. Your role will allow you to influence patient outcomes significantly through a collaborative approach to care, working alongside patients and their families while utilizing state-of-the-art rehabilitation techniques.

Key Responsibilities

• Deliver expert physical therapy services tailored to each patient's specific needs, facilitating recovery and functional enhancement in accordance with prescribed treatment plans.
• Conduct comprehensive evaluations of patients, assessing their mobility, muscle strength, joint function, and activities of daily living to establish baseline measurements.
• Carefully design, implement, and adjust individualized therapy programs, which include both manual therapeutic exercises and functional activities that aim to optimize the client's physical abilities.
• Provide thorough oversight to Physical Therapy Assistants, ensuring the implementation of care plans aligns with professional standards and is delivered with excellence.
• Regularly evaluate home environments for rehabilitation potential, making evidence-based recommendations to improve patient independence.
• Engage actively in interdisciplinary team meetings, contributing your insights and evaluations, and collaborating with healthcare professionals to create holistic patient care plans.
• Prepare and maintain accurate clinical records, including evaluations, care plans, progress notes, and discharge summaries to ensure compliance and enhance communication with healthcare teams.
• Advise clients and their families on therapeutic devices, ensuring they understand their function and maintenance as part of their ongoing care and education.
• Stay vigilant in monitoring and documenting any changes in the client's condition, proactively making recommendations for adjustments in therapy as necessary.
• Be accessible to assist Physical Therapy Assistants during client treatment sessions to ensure the highest standard of care is maintained.

Our clientele primarily consists of individuals recovering from surgery, managing chronic illness, or requiring rehabilitative support following injury. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is required.
• An active state Physical Therapist license without any history of sanctions or board proceedings.
• Exceptional mastery of diverse manual therapy modalities to effectively address patient needs.
• A proven ability to design and implement successful exercise programs tailored specifically to patient requirements.
• Familiarity with EMR software, particularly Web PT, is essential for accurate documentation and efficient patient management.
• A strong understanding of current, evidence-based physical therapy practices and techniques.

Preferred Experience

• Experience in home health care settings, which will enhance your understanding of working in varied patient environments.
• Proficiency in treatment modalities and techniques specific to the diverse needs of an older adult population.
• Strong interpersonal and communication skills to liaise effectively with clients, families, and interdisciplinary teams.

Compensation and Benefits
