Work Environment

This full-time position is onsite, focusing on a home care setting. You'll work directly with patients within their residences, making each interaction significant. Key Responsibilities

• Conduct comprehensive evaluations for patients recovering from illness, injury, or surgery, establishing personalized treatment plans based on their specific needs.
• Implement a variety of therapeutic interventions, including exercises, hands-on therapy, and the application of assistive devices, to alleviate pain and enhance mobility.
• Document and track patient progress meticulously, making data-driven adjustments to treatment plans in collaboration with other healthcare professionals as necessary.
• Educate and guide patients and their families on self-management strategies, assistive devices, and therapeutic exercises.
• Maintain up-to-date clinical knowledge by participating in workshops, professional societies, and reviewing relevant literature.

Patient Population / Client Focus

This role is vital in addressing a diverse patient demographic, including children, adults, and seniors, each with unique needs. You will engage with patients who may have undergone surgery, suffered injuries, or are dealing with chronic conditions. Your individual focus allows you to implement customized strategies that empower patients to achieve their therapy goals, enhancing their quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy (DPT) is preferred.
• Current and valid state license for Physical Therapy is mandatory.
• Excellent verbal and written communication skills.
• Proficient in using computer applications and documentation software.
• Demonstrated ability to work independently and adapt to a flexible work schedule.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy, preferably within a home care setting.

Compensation and Benefits
