Work Environment

This position is primarily onsite, delivering care in the homes of our patients. Key Responsibilities

As a Physical Therapist in Home Care, your daily responsibilities will include:

• Conducting thorough evaluations of patients in their homes to assess their physical conditions and rehabilitation needs.
• Developing tailored treatment plans that incorporate a wide range of therapeutic techniques, including exercise regimens and manual therapy, focused on alleviating pain and enhancing mobility.
• Implementing and progressing treatment plans according to individual patient needs, objectives, and response to therapy.
• Maintaining detailed documentation of treatment sessions, patient progress, and any modifications to therapy as needed.
• Regularly reassessing patients to track clinical outcomes and adjust therapeutic goals based on progress.
• Educating patients, family members, and caregivers on home exercise programs and the correct use of assistive devices.
• Staying updated on the latest industry practices by participating in professional development workshops and maintaining active membership in relevant professional organizations.

Patient Population / Client Focus

You will work with a diverse patient population that includes individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, chronic pain, and other health issues that require specialized rehabilitation strategies. This role requires not only clinical expertise but also strong interpersonal skills to build rapport with patients, motivating them through their recovery journeys.

Qualifications

To be considered for this position, candidates must possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice physical therapy in Virginia.
• Strong verbal and written communication skills to effectively educate patients and collaborate with healthcare teams.
• Proficiency in computer skills, including experience with electronic health record systems.
• A flexible and adaptive mindset to thrive in a home care setting where you may encounter varying patient needs.

Preferred Experience

While we welcome applications from newly graduated therapists, having 2 years of relevant experience in physical therapy is preferred. Candidates with prior home care experience will have an advantage in navigating the unique challenges presented by in-home rehabilitation services.
