Key Responsibilities\n

• Conduct thorough psychosocial assessments and develop comprehensive diagnosis plans based on your expertise and clinical experience.

• Collaborate with patients to create customized treatment strategies that specifically meet their emotional, mental, and social needs.

• Implement evidence-based therapeutic interventions including but not limited to solution-focused therapy, behavioral activation, and motivational interviewing, ensuring an outcomes-oriented approach.

• Work in close conjunction with interdisciplinary teams, including primary care physicians, psychologists, and other healthcare providers, to promote integrated care delivery for our patients.

• Continuously monitor the therapeutic process, assessing patient progress towards established treatment goals and adjusting strategies as needed, regardless of whether sessions occur in-person or through telehealth platforms.

• Assist in overseeing the administration of prescribed psychotropic medications, prioritizing adherence to treatment plans while watching for potential side effects.

Patient Population / Client Focus\n

The role primarily focuses on supporting patients within the hospice environment. This includes working with individuals facing life-limiting illnesses and their families, providing emotional support, counseling, and resources to enhance their quality of life during challenging times. Effective engagement with diverse patient backgrounds and needs will be essential.

Qualifications\n

• Master's Degree in Social Work from an accredited institution is required.

• A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in the State of Florida.

• A minimum of one year of experience in a clinical setting as an independently licensed social worker, providing direct therapeutic services.

Compensation and Benefits\n
