Work Environment

This position is structured onsite, providing an engaging work environment that fosters teamwork and collaboration among healthcare professionals. As a home health Physical Therapist, you will travel to various patient locations within Lakeland and the surrounding areas. This role offers a flexible schedule that includes traditional weekday hours, alongside rotating shifts on weekends to accommodate hospital needs. Your efforts will directly influence the effectiveness of patient care and rehabilitation in a community-oriented care model.

Key Responsibilities

• Conduct in-depth evaluations of patients' physical capabilities, identifying impairments, functional limitations, and health status changes due to injury or illness.
• Create and modify customized treatment programs that align with the physician's orders and patients' goals.
• Maintain accurate and timely documentation including evaluations, treatment logs, and discharge summaries to support clinical necessity and billing processes.
• Supervise and guide Physical Therapy Assistants (PTAs) and support personnel in delivering high-quality patient care, ensuring alignment with treatment goals.
• Collaborate effectively with a multidisciplinary team including physicians, nurses, and occupational therapists to track the progress and effectiveness of interventions.
• Engage in regular communication with patients and their families to educate them about their conditions and the interventions being performed.
• Provide constructive feedback to the management team aimed at enhancing departmental efficiency and workplace culture.

Patient Population / Client Focus

You will primarily work with a diverse patient population, including individuals recovering from orthopedic surgeries, neurological conditions, and age-related physical challenges. Your expertise will be vital in tailoring therapeutic interventions that promote functional independence and improve the quality of life for patients in their home environments.

Qualifications

• A Bachelor of Science degree in Physical Therapy from an accredited university.
• Current and valid license to practice as a Physical Therapist in the state of Florida.
• Strong communication skills, both written and verbal, to effectively interact with patients, families, and colleagues.
• Proficiency in computer skills for documentation and reporting purposes.

Preferred Experience

• A minimum of one year of experience in a physical therapy setting, preferably in home health or outpatient environments.
• Familiarity with patient evaluation techniques and therapeutic modalities including manual therapy and exercise prescription.

Compensation and Benefits
