Work Environment

This role is primarily based in a home care setting, where you will have the opportunity to work directly with patients in various locations around Keyport and surrounding areas. Our practice promotes a flexible work schedule, allowing you to choose your own hours between Monday and Friday. Key Responsibilities

• Evaluate and treat clients recovering from injuries, surgeries, or chronic conditions in their home environment.
• Develop individualized treatment plans that incorporate a variety of therapeutic techniques such as therapeutic exercises, manual therapy, and the use of appropriate equipment.
• Utilize clinical judgment to modify treatment plans based on the patient's evolving needs and progress.
• Document treatment sessions meticulously, tracking patient progress and making necessary updates to treatment goals.
• Instruct patients and their families on exercises, techniques, and use of assistive devices to enhance the patient's self-sufficiency and independence.
• Participate in professional development by attending workshops, training sessions, and continuing education opportunities to maintain and elevate clinical skills.
• Actively collaborate with a network of healthcare professionals, including physicians, occupational therapists, and caregivers to ensure a comprehensive approach to patient care.

Patient Population / Client Focus

Your role will focus on a diverse client base, ranging from pediatric patients to seniors, each with unique health challenges and rehabilitation goals. This includes patients recovering from orthopedic surgery, stroke rehabilitation, sports injuries, and various musculoskeletal conditions. You will be instrumental in helping patients regain mobility, independence, and overall well-being.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current state licensure in Physical Therapy for New Jersey.
• Strong communication skills, with the ability to convey complex information understandably to patients and their families.
• Proficiency in basic computer skills, including documentation software and digital communication tools.
• A high degree of flexibility and adaptability to cater to diverse patient needs in various home settings.

Preferred Experience

• While prior home healthcare experience is preferred, it is not required. New graduates are encouraged to apply, as we offer mentorship programs to assist in your professional development.
• Experience in creating and executing treatment plans using evidence-based practices is a plus.

Compensation and Benefits
