Key Responsibilities\n

• Conduct thorough assessments to determine the individual needs of patients, actively listening and engaging in the therapeutic process.

• Maintain consistent communication with supervisors, direct care staff, family members, and other stakeholders to ensure comprehensive care strategies are executed effectively.

• Provide supervisors with a detailed schedule of appointments, home visits, and ongoing observations to track patient progress and adapt care approaches as necessary.

• Coordinate home visits to conduct observations and data analysis, ensuring a holistic view of patient needs and treatment effectiveness.
\n
• Facilitate the effective implementation of program plans by engaging in appropriate therapeutic activities and adjustments based on patient feedback.
\n
• Participate actively in Interdisciplinary Team Meetings, providing insight and updates regarding patient care and progress.
\n
• Document key progress notes monthly and quarterly to accurately reflect patient trajectories and treatment effectiveness.
\n
• Lead training sessions for staff and team members to ensure everyone is competent in using program plans and documentation protocols.
\n
• Conduct follow-up interviews and home visits to offer ongoing support and modify treatment plans as patients evolve in their recovery journey.

• Maintain secure and organized patient records to comply with regulatory standards and ensure information is readily accessible to authorized personnel.

• Act as a liaison between the public, individuals seeking mental health support, and the larger mental health community to aid in program development and outreach.

• Assist in creating and refining training materials to enhance care practices within the organization.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.
