Key Responsibilities\n

As a Licensed Professional Counselor, your responsibilities will encompass a range of duties aimed at enhancing the quality of care for our clients. Key responsibilities include:

• Conducting thorough assessments of patients to identify their mental health needs and formulate effective treatment plans.

• Maintaining open communication with supervisors, direct care staff, families, guardians, and other relevant individuals regarding client progress and treatment adaptation.

• Creating and managing a tentative schedule that includes appointments, home visits, and performance observations to facilitate ongoing support for clients.
\n
• Coordinating home visits and conducting in-home assessments, chart reviews, and data analysis to ensure comprehensive care.
\n
• Executing the program plans with fidelity, monitoring implementation, and making necessary adjustments based on client feedback and progress.

• Participating in interdisciplinary team meetings to collaborate on patient care strategies and contribute insights based on clinical experience.

• Completing accurate monthly and quarterly progress notes to document client progress and treatment compliance effectively.

• Organizing training sessions to ensure that team members are knowledgeable and competent in delivering high-quality care and documentation.

• Providing follow-up care through interviews, home visits, and continuous observations to meet the needs of each client.

• Acting as a liaison between clients and the public mental health community, facilitating access to services and resources.

Qualifications\n

Compensation and Benefits\n
