Key Responsibilities\n

• Ensure the effective execution of treatment plans, regularly reviewing and modifying these as needed based on client feedback and progress.

• Participate actively in Interdisciplinary Team Meetings, contributing insights and strategies that enhance our holistic treatment approach.

• Complete insightful monthly and quarterly progress notes that reflect the patient’s journey and outcomes.

• Organize training sessions that empower staff members to competently implement treatment plans and documentation standards.

• Provide ongoing support through follow-up sessions, observations, and home visits to address clients’ evolving needs.

• Maintain and update patient records, ensuring all information is readily accessible to team members and relevant stakeholders.

Patient Population / Client Focus\n

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
