Key Responsibilities\n

• Conduct ongoing assessments to evaluate and respond to the evolving needs of the patients you serve, including crisis intervention strategies as necessary.

• Collaborate effectively with supervisors, direct care staff, family members, and guardians to communicate individual patient progress and develop tailored therapeutic plans.

• Provide a structured schedule that outlines appointments, home visits, and progress monitoring activities to ensure comprehensive support of client needs.

• Coordinating home visits to assess in-home environments and adaptability for therapeutic interventions.
\n
• Implement and manage program plans, ensuring they are effectively executed to enhance patient outcomes.

• Engage actively in Interdisciplinary Team Meetings to contribute insights and strategies relevant to patient treatment approaches.

• Maintain clear and accurate documentation through monthly and quarterly progress notes, ensuring all care plans are updated and aligned with best practices.

• Provide training and support for staff on the implementation of program plans and documentation, ensuring compliance and effectiveness.

• Conduct follow-up interviews and observations, reinforcing support and continuity of care for individual clients.

• Act as a liaison to the broader mental health community, fostering partnerships and resources to support program goals and client needs.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related field from an accredited institution.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Texas.

• A minimum of 2 years of relevant counseling experience, preferably within a clinical setting.

• A strong foundation in legal and ethical standards related to mental health practice.
