Key Responsibilities\n

• Participate in Interdisciplinary Team Meetings to foster an integrated approach to patient care.

• Complete detailed monthly and quarterly progress notes to document advancements in patient care.

• Organize training sessions to ensure team members are proficient in the necessary documentation and care implementation techniques.

• Facilitate ongoing follow-up interviews and observations to adapt care plans to evolving patient needs.

• Maintain accurate and comprehensive records to streamline information access for all involved parties.
