Key Responsibilities\n

• Conduct thorough and ongoing assessments to address the individual needs of patients, utilizing evidence-based practices.

• Maintain regular communication with supervisors, healthcare team members, families, and guardians to discuss patient progress and needed adjustments to care.

• Develop and provide a proactive schedule of appointments, home visits, and monitoring activities to track patient advancements effectively.
\n
• Coordinate essential home visits involving observations, chart reviews, and interviews to gather comprehensive data on patient needs.
\n
• Ensure the effective implementation of treatment plans, adapting strategies based on patient growth and changing circumstances.
\n
• Actively participate in Interdisciplinary Team Meetings to foster collaboration across disciplines and improve treatment outcomes.
\n
• Prepare monthly and quarterly progress notes to document patient developments and inform future care decisions.
\n
• Organize and facilitate training sessions to ensure all team members are fully equipped to implement program plans and documentation requirements.

• Provide ongoing follow-ups, including interviews and home visits, to ensure continuity of care and support

• Keep accurate and readily accessible records to share information with stakeholders involved in patient care.

• Serve as a liaison between the clinic and the broader mental health community to help articulate developmental goals and enhance program offerings.

• Assist in formulating procedural updates and training materials to continually improve our programs.

Patient Population / Client Focus\n

In this role, you will work with a diverse clientele, including adults, children, and families facing various mental health challenges. Your expertise in specialized therapies such as family therapy, couples counseling, trauma-informed care, and support for substance misuse will be valuable in addressing the unique needs of each patient.

Qualifications\n

• A minimum of a Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in California.
\n
• A minimum of two years of clinical experience in counseling is essential.

• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly preferred.

• A strong grasp of both legal and ethical counseling standards and best practices.
\n
• Exceptional interpersonal skills with the ability to establish rapport with a diverse clientele.

• Experience with Electronic Medical Records (EMR) systems and relevant technologies is necessary.
