Key Responsibilities\n

• Conduct comprehensive ongoing assessments to develop tailored treatment plans aligned with patients' unique needs.

• Develop and maintain a tentative schedule of appointments, home visits, and monitoring sessions to track the advancement of treatment plans.

• Coordinate and implement in-home observations, such as chart reviews and staff interviews, to identify ongoing patient needs and ensure compliance with care protocols.

• Engage actively in interdisciplinary team meetings, bringing clinical insights to enhance collaborative treatment efforts.

• Document and complete thorough monthly and quarterly progress notes that accurately reflect patient developments and challenges.

• Organize training sessions to certify that staff members are proficient in implementing treatment plans and utilizing documentation tools effectively.

• Support our patients through follow-up visits and interviews, ensuring continuity of care and addressing any emerging concerns.

• Maintain accurate patient records to ensure information transparency and availability for authorized parties.

• Act as a liaison with the public and other mental health professionals to provide support for community mental health initiatives.

• Assist in developing educational materials and training procedures to enhance program performance and staff capabilities.

Qualifications\n
