Work Environment\n

This position is fully remote, allowing you to balance your professional and personal life effectively. As a part of our outpatient office setting, you will engage with clients primarily through telehealth platforms. Key Responsibilities\n

• Conduct comprehensive assessments to understand each patient's unique needs and tailor individualized treatment plans.

• Maintain ongoing communication with supervisors, team members, families, and other stakeholders to monitor patient progress and adapt care strategies accordingly.
\n
• Provide timely updates regarding appointments, home visits, and progress monitoring, ensuring alignment with clinical objectives.
\n
• Perform home visits when necessary, gathering insights through direct observations, chart reviews, and interviews with families and relevant parties.
\n
• Ensure that all program plans are executed efficiently and effectively, adapting interventions as required to meet patient goals.
\n
• Participate actively in Interdisciplinary Team Meetings to collaborate and exchange insights on case management.
\n
• Prepare and file comprehensive monthly and quarterly progress notes to document advancements in patient care.
\n
• Organize and facilitate training sessions to enhance competency within the team regarding program implementation and documentation standards.
\n
• Conduct follow-up interviews and home visits to offer continued support and reassess patient needs.
\n
• Serve as a liaison with the public and mental health community, fostering connections that support the development of service goals.
\n
• Contribute to the creation of innovative training materials and operational procedures to refine service delivery.

Patient Population / Client Focus\n

You will be working with a diverse range of clients, including adults and youth facing various mental health challenges. Your expertise will guide individuals through difficulties such as trauma, substance misuse, and relationship issues, ensuring that they receive tailored, impactful support in a compassionate manner.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
\n
• Possession of active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Texas.

• A minimum of 2 years of clinical counseling experience is essential.
\n
• Demonstrated specialization in areas such as family therapy, couples therapy, substance misuse, or trauma counseling is highly preferred.

• A strong understanding of legal, ethical, and regulatory standards within the mental health profession.

• Excellent interpersonal skills with proven ability to connect with diverse clientele.

• Proficient in collaborating effectively with multidisciplinary healthcare teams.

• Experienced in using electronic medical record (EMR) systems and related technologies to enhance patient care.

Preferred Experience\n

• Specialization in evidence-based practices related to trauma-informed care.

• Prior experience in a remote counseling role is a plus.
