Key Responsibilities\n

• Conduct thorough assessments and develop personalized treatment plans tailored to individual client needs, which may include creating pre-crisis and crisis intervention plans.

• Document client interactions and treatment progress promptly and accurately, adhering to all regulatory standards and compliance requirements.

• Utilize evidence-based screening tools as recommended by the clinical team to better evaluate and support client needs.

• Collaborate with interdisciplinary teams to design and implement effective positive behavior support plans that promote positive outcomes.

• Engage actively in licensing and accreditation processes to continuously uphold the standards of care.

• Act as a liaison between clients and relevant state agencies, service systems, and community partners to ensure comprehensive support.

• Foster and maintain clear communication channels, working towards fulfilling contract standards while enhancing the organization’s outreach and educational initiatives.

• Seek opportunities for professional and personal growth by attending relevant conferences, workshops, and state meetings to share insights and represent Reliant Health Center.

• Promote and embody the agency’s mission, values, and principles in every interaction and task undertaken.

• Complete additional tasks as assigned by the Clinic Director to further support organizational goals.

Patient Population / Client Focus\n

You will work with a diverse clientele, including individuals experiencing mental health crises, substance use disorders, and those coping with developmental and acquired brain disabilities. Your role is crucial in delivering tailored therapeutic interventions that contribute to improved emotional well-being and recovery outcomes for every client.

Qualifications\n

• A Master’s Degree in Mental Health Counseling is required.

• An independent LMHC license issued by the state board in New York is essential.

• Proven ability to manage clinical records and generate comprehensive reports that reflect a meticulous approach to client care.
