Key Responsibilities\n

• Develop individualized aftercare plans for each client to ensure their successful re-integration upon discharge from services.

• Create and maintain detailed and timely documentation, including biopsychosocial assessments, treatment plans, progress notes, and discharge summaries in accordance with company policies.

• Participate in case management and care coordination, actively engaging in treatment team meetings and utilization reviews.

• Conduct comprehensive psychosocial history interviews, including assessments for suicide risk and forensic evaluations when necessary.

• Research and advocate for community resources such as food assistance, childcare services, and healthcare to support clients’ overall well-being.

As a Licensed Clinical Social Worker at Sutter Health Counseling, you will primarily work with students and their families facing various challenges. Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience.
